State Representative Workshop Registration
The February State Rep Workshop will be held in Adelaide for 2 days, Saturday 4th - Sunday 5th and will include an official State Rep Dinner on the Saturday night. A full timetable for the workshop will be sent to all registered State Representatives with their itineraries one week prior to the workshop.
Communication
All communication will be via the email address provided in this form. It is your responsibility to keep your email address current via rayad@ayad.com.au. The AYAD Program will contact you immediately on receipt of this form to confirm your attendance at the workshop and again one week prior to the workshop with your itinerary and a full program.
Flights
All SRs from outside Adelaide will be flown in on Friday night the 3rd February (after 5pm) and return flights will be booked for Sunday the 5th February (from 10:00am for 2011 State Reps).
If you wish to extend your stay please specify the dates and times you wish to fly in and out of Adelaide in the notes section on the form. If you wish to extend your stay or change your flights after receiving your travel itinerary it must be done directly with the booking agent and you will need to pay any extra costs incurred.
Tickets will be electronic (E Ticket) and you will be required to show some photographic identification at the airport.
Accommodation
Accommodation will be provided for Friday and Saturday night (including Adelaide based SRs, unless notified otherwise). Accommodation will be single rooms (not twin share).
Meals
The AYAD Program will provide all meals for the duration of the workshop.
Airport Transfers
Airport transfers will be provided in Adelaide from the airport to the hotel and return. Transport to the airport from your home and on return can be reimbursed by the AYAD Program. Please send your receipts to RAYAD, c/o Austraining International, Level 1/41 Dequetteville Terrace, Kent Town, SA, 5067 and include your full name and AYAD intake and the reimbursement will be deposited into your bank account.
Cancellation
If, after registration, you need to cancel your attendance at the workshop please contact Betty Cherian via rayad@austraining.com.au. If less than 72 hours notice is given you will be liable for the cost of your flight.
